Patient aged 55 with problems with the eyes, who went to see the Optician. He suggested there was a problem with the back of the eye and needs assessment. Patient then takes the optician supplied GOS18 form to see the GP. As per protocol, GP then immediately refers the patient to be seen in the Community ophthalmology clinic at a convenient time (after the patient has finished work in a community clinic at 7 o’clock in the evening). The patient feels pleased that he was contacted and appointment provided and seen within five working days by a NHS consultant close to his home in a GP surgery. His condition was diagnosed as a mild problem, explained , treated and reassured and fully dealt with and the medical report has been immediately faxed to his GP with a copy provided to the patient.
